UNLIMITED CONSENT AND RELEASE FORM

The undersigned model and photographer agree to accept all conditions set forth in this 
unlimited consent and release.

I, the undersigned model, give the undersigned photographer exclusive permission to sell, 
publish, copyright, distribute and/or display photographic images of me taken on the below 
listed date.

I, the undersigned photographer, agree to compensate the undersigned model as detailed below.

I, the undersigned model, understand that the photographer  may use any professional or artistic 
process or procedure resulting in the completion of the finished product when photos of me are 
used.

USAGE AUTHORIZED WITH THIS RELEASE: Unlimited use, including but not limited to:  
Portfolio and public art gallery displays of prints. Posting on the undersigned photographer's 
web site, Internet forums, or other online submissions for publicity and promoting 
photographer's web site or services; contests; paper media advertising and marketing of the 
photographer's services; sales to commercial and stock photography companies, magazines, 
periodicals, individuals and galleries.

COMPENSATION AGREED TO WITH THIS RELEASE: The undersigned model will be 
allowed to review all images featuring the undersigned model and created by the undersigned 
photographer on the date indicated below. In addition, the undersigned model will receive 
low resolution copies in a web page format on CD and one 8"x10" photo print per hour 
modeled, chosen by the undersigned model, or equivalent as agreed to by both parties.  The 
model may use these prints/copies and digital images for self promotion and non-commercial 
purposes.  Additional prints and high resolution images may be purchased separately.

By signing this unlimited consent and release form I, the undersigned model, represent that I am 
of legal age in the state in which this form is executed; that before signing this document, I have 
read it completely and I understand it.
